This form documents the written consent provided by Lena Health employees and authorized contractors who receive SMS text messages during development and testing of the Lena Health patient-messaging application. A copy is published at this address for messaging campaign verification purposes.
Consent to Receive Test Text Messages
I voluntarily consent to receive SMS text messages from Lena Health (Bloom Circle, Inc.) at the mobile phone number I provide below, for the purpose of developing, testing, and validating Lena Health's patient-messaging application. I acknowledge and agree to the following:
- The messages I receive will be representative of Lena Health's production care-navigation message content and are sent to me for testing purposes only.
- Message frequency varies during active testing periods.
- Message and data rates may apply, depending on my mobile plan.
- I may opt out at any time by replying STOP to any message, and I may reply HELP for assistance.
- My mobile phone number and text messaging opt-in data and consent will not be shared with or sold to third parties or affiliates for marketing or promotional purposes.
- My participation is voluntary, and I may withdraw at any time without any effect on my employment or engagement with Lena Health.
- I have been provided with Lena Health's Privacy Policy and Terms of Service, which include the SMS program terms.
